For Patients

If you're a patient and you already have a referral form from another doctor, you can fax or email your referral to our clinic.

For Doctors

Cancellations & Missed Appointments Policy

Our clinic manages a significant demand for appointments on a daily basis. If you are unable to attend your appointment, please inform us 72 hours in advance to allow access to another patient. Failure to attend your appointment creates considerable delay in the provision of care for yourself, and prevents access to care for other patients in need. 

Cancellations should be done during regular business hours with a staff member. As such, missed appointments without prior notice will be subject to the clinic’s cancellation fee of $75.00.

Once your appointment is booked, you are responsible for noting the date and time. If you have an outstanding balance, it must be paid prior to your next appointment with your doctor. Patients with a repetitive cancellation history may be denied future appointments.
